Job Summary Performs biopsycho-social assessments, creates and updates individualized treatment plans, provides individual, family and group therapy for patients to Outpatient Behavioral Care Services. Specializes in work with adults, and/or adolescents, and families experiencing mental health/substance abuse problems. Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Serve with compassion. Be the difference. Completes an accurate biopsychosocial assessment, including competency in diagnostic determinations and determination of appropriate level of care for patient. Provides direct and indirect clinical/therapeutic services to patients and their families with clinical supervision as needed. Develops and individualized treatment plan including measurable treatment goals related to patient's presenting problems. Participates as an active member of the multi-disciplinary treatment plan. - Provides the most appropriate therapy, specific to patient's symptoms. Performs any case management responsibilities involved with direct patient care. This includes, but is not limited to, Utilization Review, Discharge Planning, Family Involvement/Education, timely and appropriate documentation of care provided, communication with referral sources, completion of short-term disability paperwork and communication with patient's employers as well as coordination of care with multidisciplinary treatment team and outside community providers. Participates in outcome monitoring activities. Performs other duties as assigned.
