About The Position

Supports individuals to achieve their deserved quality of life, and ensures optimal health and safety by providing prescribed behavior support and supervision ECF-Community Living Supports Department, Supported Living Division

Requirements

  • High School Diploma or GED and 5 Years experience working with individuals with Intellectual Disabilities and Mental Health Diagnosis
  • OR an Associate degree in a related field and 3 years experience working with individuals with Intellectual Disabilities and Mental Health Diagnosis
  • OR a Bachelor degree in a· related field and 1 year experience working with individuals with Intellectual Disabilities and Mental Health Diagnosis
  • The ability to read and write the English language to communicate important information efficiently is required.
  • The documentation necessary to complete an 1-9 form is required.
  • An acceptable motor vehicle record is required for those seeking or working positions that require driving, in addition to a valid drivers license. An F endorsement is required for licenses from the State of Tennessee, though time will be allowed to obtain this after employment begins. Employees driving their own vehicles for work purposes must also have the required liability insurance.
  • Basic computer skills are required.
  • Passing the required physical examination by an Orange Grove Center approved physician is required.
  • The ability to perform the duties noted within the Work Capacity Requirements Addendum is required.

Responsibilities

  • Support individuals to meet there individual goals
  • Monitor and record data on personal goals
  • Provide the appropriate level of support each situation requires according to the Person Centered Support Plan (PCSP), Behavior Plan/staff instructions, Physical and Nutritional Plans, seizure protocols, other care plans/instructions; and health status
  • Support individuals with choice making
  • Support individuals in their work, social, and leisure activities
  • Support family relationships as appropriate
  • Support community integration
  • Remain on shift until the relief staff arrives
  • Be attentive to and report any health and safety concerns of individuals
  • Ensure the safety of all individuals in cases of emergency events, such as-but not only-fire, tornado, and armed intruder
  • Follow emergency procedures accurately and promptly, such as CPR, First Aid, Heimlich Maneuver, and Crisis Prevention (CPI)
  • Observe and record accurate information on seizure reports
  • Utilize Standard Precautions for proper protection and hygiene
  • Support individuals at medical and behavior specialist appointments, including providing reliable verbal and written data
  • Support positive lifestyle choices
  • Administer medications accurately in accordance with the PCSP, state, company, and Departmental policy and procedures
  • Keep/store all chemicals and cleaners in the manner required by PCSP, regulations and department procedures
  • Complete all required documentation per shift
  • Perform all personal hygiene in a private setting
  • Assist with bathing, hair care, dental hygiene, and shaving
  • Assist with nail care
  • Assist with dressing and undressing
  • Assist individuals with selecting appropriate, clean and seasonal clothes
  • Assist with toileting, including with disposable adult briefs if necessary
  • Utilize mechanical lifts and all other equipment necessary according to individual's program plan and BSP.
  • Utilize proper lifting techniques in all areas
  • Ability to lift, move, and position individuals according to therapists' instructions and individuals' program plans.
  • Ability to stoop, bend, and squat
  • Ability to push, pull, reach and stretch
  • Ability to run as needed in emergency situations
  • Ability to work in noisy environments, indoor and outdoor as needed.
  • Wash and dry individuals' clothing and household linens
  • Fold and iron clothes
  • Place clothing in appropriate locations for each individual supported
  • Maintain clothing inventories
  • Prepare snacks and meals per individuals' guidelines
  • Clean Kitchen, eating area, and wash dishes after meals
  • Sweep, mop, and empty trash daily
  • Clean major appliances as needed
  • Check expiration dates of food and discard expired items
  • Keep bathrooms stocked with towels, soap, and toilet paper
  • Clean sink, toilet, shower/bathtub, counter, mirror, and equipment daily
  • Sweep, mop and empty trash daily
  • Sweep, mop, vacuum, dust daily
  • Empty trash daily, and recycling receptacles as required
  • Clean trash cans as needed
  • Take trash cans to street per pickup schedule
  • Maintain perimeters of site e.g. patios, driveways, carport)
  • Disinfect common and shared surfaces and items daily
  • Be alert to and report maintenance needs and safety concerns
  • Clean and care for wheelchairs and cushions daily
  • Change air filters as needed, per discretion of department protocol
  • Unstop backed-up toilets
  • Ensure wheelchair and mechanical lift batteries are charged daily.
  • Clean and care for all heavy equipment and machinery as required and per protocol
  • Be alert to and report any equipment and machinery needs or safety concerns immediately
  • Store and dispose of biohazard materials per company policy
  • Operate vehicle per local, state, and federal laws, and per OGC policy
  • Purchase fuel for vehicles per OGC fueling guidelines
  • Safely secure passengers who use wheelchairs in vehicles per OGC policy
  • Operate wheelchair lifts safely per OGC policy
  • Support individuals with arranging transportation for appointments and events
  • Become familiar with local community routes, roads, major sites and attractions.
  • Maintain a valid license with F endorsement (for Tennessee licenses)
  • Remain current with all company required drivers' training
  • Drive safely per environmental conditions and defensive driving guidelines
  • Keep exterior and interior of vehicle clean
  • No eating, drinking, or smoking allowed in company vehicles at any time.
  • No use of cell phones or other electronic devices while operating an OG C vehicle, per OGC policy
  • Keep fuel tank at least half full at all times
  • Know the location and proper use of safety equipment present in any OGC vehicle being operated (e.g. First Aid Kit, fire extinguisher, we cutter, triangles, etc.)
  • Complete daily pre-trip and post-trip inspections to ensure vehicle is safe and legal to drive
  • Complete post-trip inspection after every trip to ensure that no passenger is ever left in a vehicle unattended
  • Comply with OGC Check Seat Sign policy for every trip, to ensure that no passenger is ever left in a vehicle unattended
  • Only use personal vehicles to transport individuals when currently working within a department that allows use of personal vehicles
  • Maintain a valid license with F endorsement (for Tennessee licenses), and liability insurance; and inform company immediately if license or insurance is revoked, suspended, or expired
  • Become familiar with local community routes, roads, major sites, and attractions
