JOB PURPOSE: Coordinates all aspects of the developmental, implementation and evaluation of the efficacy of individualized interventions, which are designed to address the priority needs of the clients on his/her caseload. KEY RESPONSIBILITIES: Compiles data from Level 1 training programs, toileting data, and behavioral data. Conduct 1:1 dining training. Conduct 1:1 training programs. Implements behavioral management techniques when inappropriate social behaviors are exhibited by any client. Conducts team activities. Conduct daily activity probes on daily team/client activities. Distributes data forms: Places B & B sheets in client rooms. b) Place contingency procedure forms in DCS data notebooks Places behavioral data collection sheets in DCS notebooks Updates Task Analysis sheets. Attends IDT meeting as scheduled. Gathers behavioral data (incidents) for medication review team. Post on exemption board clients for medication review. Attend quarterly reviews as requested. Conducts annual assessments for assigned teams. Keep DCS notebooks current on Living units (North and South). Files new programs and IPP in DCS notebooks (North and South) and in Level book (in Active Treatment Team offices.) Places skill acquisition sheets, monthly calendars, and required relaxation sheets in DCS notebooks on Units (North and South) Provides in-service training to Direct Care Staff and be accountable to implement data and training procedures Operates at all times within ICF/MR regulations, applicable federal and state laws, and facility policies.
