General Summary: The Behavior Coach role will provide intensive direct support services to families who have children that are “at-risk” by constructing programming that is unique to each client. A Behavior Coach will provide family, school-based interventions, and community centered interventions, which are guided by positive behavior support principles that address the needs identified by the CFT, the family, and the school. The Behavior Coach is responsible for meeting their clients where they are in the home, school and community, helping to support individuals in developing positive behavioral strategies to improve their social, emotional, and/or academic success. This role involves working with children and adolescents and their families and CFT teams to create and implement individualized behavior intervention plans and promote a supportive learning environment. Manage client caseload, review individualized service plans and maintain chart compliance for all clients.
