About The Position

This position supports Mercy's philosophy of patient centered care by assisting in the coordination of care of patients with cancer. The Patient Navigator - CMA/RMA assists in expediting the process for evaluation of cancer patients through a multidisciplinary team approach, coordination of services and scheduling exams, optimizing patient care outcomes. Will also be responsible for providing direct patient care.

Requirements

  • Strong computer skills required
  • Proficiency with utilizing Microsoft apps (i.e. Excel, Word, PowerPoint, etc.) or the ability to learn required
  • Excellent communication and customer service skills, computer skills, organization, time management, attention to detail and the ability to remain poised and efficient in a fast-paced environment.
  • Stays updated on bariatric program protocols, guidelines, and best practices to provide informed guidance to patients.
  • Willingness and ability to help foster a culture of constructive feedback and continuous improvement among peers, promoting a supportive and non-judgmental environment.
  • Willingness and ability to provide cross-coverage to fellow front office or back-office teammates when needed/requested.
  • Education sufficient to obtain required certification.
  • Certified or Registered Medical Assistant required.

Nice To Haves

  • One year experience working in a medical office setting preferred.

Responsibilities

  • Maintains departmental work queue, coordinates appointments with providers to ensure timely delivery of office visits, diagnostic and treatment services.
  • Works within their scope of practice to navigate the patient through their cancer care.
  • Communicates effectively with internal and external physicians and staff.
  • Works closely with patients and their families to identify barriers to care. Assists with navigating those barriers when applicable.
  • Assists with patient rooming and care coordination during Multi D clinics, to include vital signs, health history, med list, order entry, and referrals.
  • Prepares patients for physician exam. Instructs patients as directed by the RN or physician.
  • Assists with patient phone calls, scheduling, and medical record management.
  • Provides after visit summary and completes discharge of patient from the exam room, including follow up instructions, scheduling, and referrals.
  • Identifies and coordinates patients for cancer conferences and multidisciplinary cancer clinics. Assists with cancer screening among clinics and Mercy.
  • Facilitates multidisciplinary clinics.
  • Follows Mercy's safety guidelines, carries out job-specific safety duties and responsibilities, and promptly reports any unsafe conditions, situations, incidents and injuries.

  • Coordinates patient appointments with providers and other clinics to ensure timely delivery of care specific to the Bariatric Surgery Program patient pathway.
  • Serves as a healthcare liaison between patients and the Bariatric Surgery Program.
  • Coordinates services internally and externally to assist the patient through the Bariatric Surgery Program.
  • Obtains prior authorizations for patients as needed.
  • Provides education and referral options to patients, placing an emphasis on services offered at Mercy Medical Center.
  • Serves as the Bariatric Surgery Clinic representative by providing patients with accurate and timely information as directed by the Bariatric Surgery Director and coordinator.
  • Work closely with all members of the Bariatric Surgery Clinic team to assist with the fluidity of day-to-day bariatric surgery program operations.
  • Collaborate with the Bariatric Program Coordinator, Bariatric Program Director, Bariatric Nurse, Bariatric CMA, and Clinic Manager to ensure smooth operations and adherence to program goals and objectives.
  • Maintains the bariatric retail area, including inventory management, restocking, and assisting patients with purchasing retain items
  • Serves as a welcoming committee for patients, providing a warm and friendly environment upon their arrival

  • Provides support and guidance to patients throughout their journey, addressing their concerns and questions while staying within the CMA scope of practice.
  • May assist with educational programs and facilitating support groups for bariatric surgery patients.
  • Maintains patient records and documentation, ensuring confidentiality and accuracy.
  • Participates in staff meetings, trainings, and professional development activities as required by program and clinic leadership.
