About The Position

The Autism Services Navigator (ASN) plays a crucial role in supporting TRICARE beneficiaries enrolled in the Autism Care Demonstration (ACD). This position involves collaborating with families to assess, plan, facilitate, and coordinate care for individuals diagnosed with autism spectrum disorder (ASD). The ASN serves as an advocate, helping families navigate barriers to care and access necessary services, ensuring comprehensive health needs are met effectively and efficiently.

Requirements

  • Current unrestricted licensure as a Licensed Clinical Social Worker, Registered Nurse, or other licensed mental health professionals with a Certification in Case Management (CCM).
  • Three years of full-time equivalent direct clinical care experience.
  • Clinical experience in pediatrics, behavioral health, and/or ASD; healthcare environment; and proven care management experience.
  • Ability to obtain an industry-recognized certification in case management within four years of hire.
  • Must be able to receive favorable Interim and adjudicated final Department of Defense (DOD) background investigation.
  • U.S. Citizenship.

Nice To Haves

  • 2 years' experience working with individuals diagnosed with ASD and/or the TRICARE ACD.
  • 2 years case management experience.
  • Possess industry recognized case manager certification.

Responsibilities

  • Collaborate and oversee assessment, planning, facilitation, care coordination, evaluation, and advocacy for TRICARE beneficiaries diagnosed with ASD.
  • Educate beneficiaries on ACD and TRICARE benefits.
  • Complete comprehensive case management assessments and develop individualized comprehensive care plans (CCP).
  • Reassess and monitor CCPs regularly to address newly identified needs and document changes in the medical management system.
  • Evaluate case management outcomes and assess beneficiary satisfaction with the care plan.
  • Function as an advocate for beneficiaries by ensuring access to necessary care and locating specialized services.
  • Support beneficiaries and families by providing TRICARE and community resource information.
  • Coordinate and participate in Medical Team Conferences with civilian and military providers.
