About The Position

Employees supervise the provision of support coordination services to people who have intellectual and developmental disabilities or autism to assist them in attaining a more inclusive social, economic, emotional and physical life while maintaining health and safety. Work involves responsibility for supervising and training a small group of professional county social service workers, but the worker may also carry a caseload. Work is performed in accordance with established regulations, policies and procedures, but employees are expected to exercise considerable initiative and independent judgment in achieving objectives. Work is reviewed by a professional or administrative supervisor through conferences, reports and results attained. This position may require work beyond the 37.5 hours of work per week, depending on the demands of the work.

Requirements

  • Two years of professional experience in public or private social work and a bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences or Any equivalent combination of experience and education which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.

  • Team Leadership - encourages open dialogue, builds morale based on knowledge of individual strengths and weaknesses, shares wins and pushes tasks and decisions down to let each support coordinator take an active role in project completion.
  • Intellectual Disability & Autism- must have advanced knowledge of field of expertise, as well as related fields, e.g. Medicaid Waiver, Mental Health, Person-Centered Planning, Self-Determination, etc.
  • Must pass pre-employment drug screening test.
  • Must have acceptable background check that includes FBI Clearance, PA State Police criminal history and ChildLine clearances. Clearances are required prior to the start of employment and may not be more than one year old at time of employment.
  • A valid driver’s license and acceptable driving history in accordance with County policy.
  • Assistant Chief Caseworker must be able to work in a busy and open environment.
  • Routinely provides own transportation to and from consumer's homes, meetings, outreach visits and other job-related appointments in a timely manner and is able to access those locations with reasonable accommodation.
  • Assistant Chief Caseworker must be able to use a computer for data entry.

Responsibilities

  • Coordinates and reviews case records, documents and information prepared by staff to ensure that they are in compliance with agency policies and procedures and legal requirements.
  • Provides case management services to people with intellectual and developmental disabilities
  • Motivates staff to develop and use their own potential for successful completion of their job tasks.
  • Reviews and evaluates the work of subordinate staff.
  • Communicates with individuals/families/providers/community members regarding work of subordinates and functions of the agency.
  • Meets with representatives of other community agencies and agency staff to coordinate services.
  • Reviews and monitors client records to assure services are provided in compliance with applicable regulations.
  • Prepares social studies, case summaries, reports, and correspondence.
  • Prepares and conducts training to both individual support coordinators and groups of support coordinators as needed.
  • Interviews potential job candidates and conducts reference checks.
  • Assists with hiring new support coordinators
  • Performs related work as required.
  • The employee will work in collaboration with the Director(s) of the unit and Administrative Entity (AE) staff to promote a cooperative work effort within the Support Coordination Unit.
  • They will promote a cooperative work effort within BHDS's various components, e.g. administrative, mental health, as well as contracted service providers and the community at large.
  • The employee will attend and participate in management and other meetings as needed.
  • The employee will support business decisions of the agency and work to positively promote changes to support coordinators.
