Requirements

  • Current, unrestricted licensure of Doctor of Medicine or Osteopathy.
  • Must be able to obtain physician licensure in the State of Ohio at time of start of employment.
  • Must maintain a valid DEA license and board certification.
  • Initial board certification in a primary ABMS specialty. Preferred primary specialties of oncology, internal medicine, family practice, hospitalist medicine, or geriatrics.
  • Minimum 2 years of demonstrated work experience in hospice and/or palliative care or another similar setting managing the care of terminally ill patients.
  • Reliable transportation, a valid driver’s license, a good driving record and auto accident insurance at a level acceptable to the agency’s insurer are also required.
  • Basic computer skill.
  • Ability to plan, organize and direct a variety of administrative functions.
  • Ability to develop plans to meet organizational and team goals.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to communicate complex ideas and issues, both orally and in writing.
  • Ability to supervise, evaluate and train assigned staff.
  • Ability to provide leadership and serve as a hospice and palliative care ambassador within the community.

Nice To Haves

  • Experience caring for patients in a hospice inpatient unit setting preferred.
  • Board-certification or Board-eligibility in Hospice and Palliative Medicine, PREFERRED.
  • Applicants currently participating in a qualified Hospice and Palliative Medicine Fellowship may apply.
  • If Board Eligible, Board Certification in Hospice and Palliative Medicine is required within 2 years of employment.
  • Alternatively, Hospice Medical Director Certification with high-level work experience in the field may be considered in lieu of fellowship training and certification.

Responsibilities

  • Consistent with 42 C.F.R. § 418.64 (a) Physician acknowledges that he/she is, along with the Hospice Medical Director, Hospice’s physician employees, contracted physicians, and each patient’s attending physician, responsible for the palliation and management of the terminal illness and conditions related to the terminal illness of Hospice's patients.
  • Physician will perform administrative and patient care services as requested by Hospice or the Hospice Medical Director, which may include, but are not limited to, the services described in this document.
  • Physician, as a physician member of the hospice IDG, will perform certification/recertification of terminal illness consistent with 42 C.F.R § 418.22 (c).
  • Physician will participate in IDG/transdisciplinary team conferences to develop and revise patient care objectives and procedures; provide expert consultation and participate in team decisions regarding appropriate settings and levels of care for patients; and collaborate with pharmacists concerning medication formulary and best prescribing practices.
  • Physician will perform face-to-face encounters, as needed, along with the performance of other clinical aspects related to the Hospice's patient care program, including coordination of admissions and discharges.
  • Physician will provide direct medical care to patients in their home setting or in the hospice residential facility, which may include being the patient’s designated attending physician if requested by the patient or the patient's representative.
  • Physician will keep abreast of standards and regulations, attend trainings, participate in QAPI, and meet all other regulatory and agency requirements.
  • Physician will develop and maintain positive relationships with the physician community, promoting hospice and palliative care; precepts medical trainees.
  • Physician will participate in Hospice's committee work, as requested.
  • Physician will participate in and/or perform palliative care consultations, as requested.
  • If serving as the Physician Designee, Physician will assume responsibility of the medical component of the Hospice's patient care program pursuant to the Physician Designee Policy and the Appointment of Physician Designee form. Lead Physician will not assume the Physician Designee role without direction from the Hospice or the Hospice Medical Director.
  • Physician will perform other duties, as required.
  • Ability to lead a team for inpatient hospice care and Care Centers for Reserve Care
  • Primary physician lead as inpatient rounder.
  • Attending for residential patients as required.
  • Weekend support as needed
