About The Position

The ACSW / AMFT will provide assessment, diagnosis, treatment, crisis intervention, case management for members with mild to moderate behavioral health needs; formulate treatment plan, conduct psychotherapy-treatment, and referral of members with acute or chronic psychiatric and/or behavioral health needs. Collaborates with treating physician to plan and direct each patient’s treatment program through collaborative care model.

Requirements

  • Holds a MSW from an accredited school of social work, ASW required or Master degree from accredited MFT program, AMFT required. school of social work, ASW required or Master degree from accredited MFT program, AMFT required.
  • Experience with one-on-one patient health
  • Excellent written, verbal, and interpersonal communication skills
  • Has great interest in the implementation of integrating primary care and behavioral health care.
  • Current documentation of Basic Life Support is required.
  • Must be able to fluently speak, read and write English.
  • This is an FLSA non-exempt position.
  • This is an OSHA high-risk position.

Nice To Haves

  • Fluency in Chinese (Cantonese and/or Mandarin), Vietnamese, Spanish and other languages may be required depending on location.
  • Fluency in other languages are an asset.

Responsibilities

  • Provide culturally sensitive, linguistic appropriate assessment.
  • Follow NASW code of ethics.
  • Performs a bio-psychosocial assessment interview with patient according to department policy and procedures.
  • Formulate treatment plan, continue to engage in patient to improve behavioral health conditions.
  • Under LCSW supervision, attend weekly clinical supervision with LCSW; participate in department monthly case consultation.
  • Provide appropriate crises intervention/treatment to adult patient in emergent situations including assessment, counseling, information/referral and providing consultation to physicians and healthcare team.
  • Maintain a regular case load.
  • Provide Warm Handoffs between primary care providers and NEMS patients.
  • Documents treatments and progress in accordance with state regulations and department procedures.
  • Supports and counsel’s patients with psychoeducation, regarding anxiety, depression, insomnia and compliance with prescribed therapeutic regimens, including adherence to prescribed medication regimens within the scope of practice.
  • Communicates concerns about medications or side effects to PCP/NP/psychiatrist.
  • Submits timely billing record to the billing department to enable billing for behavioral health services.
  • Performs other job duties as required by the specialty or by manager/supervisor.
