About The Position

As an Associate Health Consultant at HealtheMed, you will play a vital role in our hybrid care model, providing hands-on support to clients and ensuring they thrive in our innovative system. This position involves conducting in-person visits to assist clients in utilizing HealtheMed's technology platform and remote patient monitoring devices, while also supporting them during telehealth appointments. Your work will focus on improving clients' overall health and wellness by guiding them through their health journey and empowering them to take charge of their health.

Requirements

• Graduate of an accredited EMT program
• Certification by the State of Minnesota as an EMT
• Current Driver's License with a clean driving record
• 2 years of hands-on EMT experience in the field
• At least 1 year experience working with underserved populations, such as those on the Medicaid waiver program.

Nice To Haves

• Experience working in community health or outreach roles
• Familiarity with telehealth platforms and/or remote patient monitoring devices
• Strong communication skills to build trust with clients from diverse backgrounds
• Self-motivated and organized with the ability to adapt to a dynamic work environment.

Responsibilities

• Perform in-person visits to clients' homes to provide education on HealtheMed's technology platform.
• Ensure proper use and functionality of remote patient monitoring devices.
• Support clients during telehealth appointments by ensuring connectivity and addressing questions.
• Act as a bridge between clients and the care team, ensuring clients are engaged in their care plans.
• Provide on-the-spot troubleshooting for technology-related issues.
• Escalate client concerns to appropriate care team members and provide resources to clients.
• Maintain accurate clinical and business documentation in EMR and CRM systems.
• Think strategically for organizational solutions and during client coaching.
• Maintain education and competencies to provide high-quality patient care.
