This is an exciting opportunity to join our diverse team in the following role. If you are interested in joining our team, apply today! The Population Health Management Division (PHMD) plays a pivotal role in designing and driving the implementation of DHCS' flagship initiative, California Advancing and Innovating Medi-Cal (CalAIM)-driving meaningful change in a start-up-like environment. PHMD is part of DHCS' Quality and Population Health Management Program (QPHM) and, as a three-year-old division, continues to grow and evolve. PHMD is currently recruiting a nimble, dynamic, and analytical individual to join the Enhanced Care Management (ECM) Unit.
