About The Position

Renaissance Social Services, Inc. (Renaissance) is a leader in Chicago’s response to homelessness. Renaissance prevents and ends homelessness for vulnerable Chicagoans, providing a range of proven solutions including prevention, outreach, rapid re-housing, permanent supportive housing, behavioral health care, and clinical case management. The Associate Director of Outreach provides oversight and leadership to the PATH and PATH-PSH staff who provide comprehensive, individualized support services using a client centered harm reduction, trauma informed model for participants enrolled/pre-enrollment in PATH and PATH-PSH Programs. The PATH program includes outreach to street and shelter-based participants across Chicagoland area, entering/exiting people to Coordinated Entry System as a skilled assessor, accompanying participants in applying for and going to medical & benefits appointments and connecting participants to case managers and community-based resources as desired by the participant.

Requirements

  • Must meet the definition of a Mental Health Professional (MHP): A Bachelor’s degree in a related human service field (Social Work, Psychology, Counseling, etc.) OR Bachelor’s degree in any other field plus 2 years of documented clinical experience in a mental health setting OR High School Diploma or GED plus 5 years of documented clinical experience in mental health or human services.
  • Minimum 3 years of experience in outreach, case management, or homeless services.
  • Minimum 1 year supervisory experience.
  • Must demonstrate competency in conducting assessments in alignment with SUPR and PATH program requirements.
  • Valid Illinois Driver’s License and a reliable, insured automobile.

Nice To Haves

  • Qualified Mental Health Professional (QMHP) Status.
  • Master’s degree in Social Work, Counseling, or Psychology.
  • Experience with HMIS and PIMSY.
  • CADC (Certified Alcohol and Drug Counselor) or CRSS (Certified Recovery Support Specialist).

Responsibilities

  • Assuming a leadership role within the PATH and PATH-PSH Teams providing case management services including intakes, assessment, mental health services, outreach, and referrals/linkage, case management assistance to participants experiencing homelessness.
  • Implementation of evidence-based practices.
  • Models evidence-based practices to entire team.
  • Ensuring that a minimum of 60% of services are provided in a natural setting.
  • Conducting assessments of participant functioning, especially as it relates to physical health, mental illness and substance use disorder.
  • Identifying participant needs, including barriers to receiving appropriate services and assisting with effective problem solving to address issues.
  • Actively gathering participant information and assesses the need for services, makes recommendations for treatment, identifies and investigates existing natural resources and supports, explains options available to the participant, and assists in application process.
  • Actively leads and oversees the activities of team members providing outreach, case management, social, vocational, education, physical well-being, and street outreach services, other services and support.
  • Meets required documentation/reporting within assigned time frames.
  • Monitors and coaches' staff on meeting productivity measures.
  • Maintains the quality, validity and confidentially of individual case records according to program requirements and CARF, regulations and agency policies and procedures.
  • Assures that all assigned documentation is completed accurately, on time and is organized in a systematic manner.
  • Assists with special tasks as assigned, including but not limited to representing the agency at meetings, conferences, and supervising students/interns, orienting and training new staff.
  • Actively participates in HMIS, SITs, SUPR, H1st, and All Chicago meetings and events in collaboration with assistant team lead and Director of Outreach.
  • Participates in the hiring sequence of the agency and makes recommendations on the hiring or termination of team staff.
  • Providing service planning, coordination, and implementation of services needed.
  • Providing information, linkage, referral and advocacy for participants with agencies in the community, including crises intervention services when needed.
  • Working with participants to increase their skills and income, improve their self-determination, and assist with obtaining and maintaining housing.
  • Reporting all incidents of abuse, and/or neglect as required by state law.
  • Follows agency standards regarding clinical supervision.
  • Conducts weekly Team meetings.
  • Continuously update participants locations, demographics, and reporting details in HMIS and PIMSY.
  • Model professional behavior for Staff.
  • Continue to attend outside Clinical Education Trainings.
  • Completing random file audits/reporting on a monthly basis.
  • Participate in quarterly Case Reviews.
  • Additional duties not listed may be required depending upon the needs of participants and at the discretion of your supervisor. Including but not limited to emergency outreach, and annual PIT.
  • Must have the ability to relate appropriately and effectively with participants, to other staff and to personnel of other community agencies.
  • Establish and maintain effective working relationships, ability to work independently and as a member of an interdisciplinary team.
