Housing for Health at NYC Health + Hospitals (NYC H+H) recognizes that the chronic health issues of unhoused individuals cannot be treated without stable housing. This initiative seeks to improve the health and wellbeing of New Yorkers by focusing on four strategic areas: housing navigation services, medical respite beds, affordable housing on hospital property, and social service support for patients in permanent housing. Housing for Health is seeking a Clinical Assessor (LCSW or RN) to work within the Office of Managed Care at Central Office under the supervision of the Director of Housing Clinical Support Services. The Clinical Assessor will support NYC H+H's flagship medical respite program for patients experiencing homelessness who have clinical needs and cannot be adequately served in a shelter. Housing for Health contracts for two medical respite programs with two non-profit vendors to deliver up to 90 days of interim housing that allows patients to receive onsite clinical services, with a goal to transition patients to permanent housing where possible. The Clinical Assessor will be responsible for performing clinical review and assessments to ensure appropriate patients are accepted by medical respite and providing warm handoffs to the respite operators. The candidate will also support the Housing for Health housing navigation program, assisting with referral review, prioritization and ensuring the creation of safe clinical housing plans. The role will subsequently assist housing navigation vendors to implement housing plans. Housing for Health is seeking candidates with strong clinical skills and demonstrated experience working with people experiencing homelessness who have complex medical and/or behavioral health conditions. Successful candidates should also possess strong connections to the network of community-based organizations (CBO), and social service supports for patients experiencing homelessness in NYC, and exceptional communication and organizational ability.
