About The Position

The Associate Director of Community Health and Outreach supports the development, coordination, and execution of CommuniCare+OLE’s community health and outreach strategy across Napa, Solano, and Yolo Counties. This role assists in overseeing community health and outreach programs and provides direct support in managing outreach staff, including Promotoras. The ideal candidate excels at partnership-building, program coordination, and community engagement, and brings experience serving diverse, multilingual, and underserved populations. Requirements

  • Education: Bachelor’s Degree OR a combination of academic education, professional training or work experience, which demonstrates the ability to perform the duties of the position.
  • Minimum of 5-7 years of direct and relevant experience or relevant college degree.
  • Experience/Lived Experience: Experience as a Community Health Worker or Promotora strongly preferred.
  • Special Skills/Training: Bilingual English/Spanish required.
  • Community outreach experience, especially with a community health center or community-based organization, strongly preferred.
  • Experience in a similar role with a strong track record of relationship‑building in low‑ to mid‑income and diverse communities.
  • Demonstrated commitment to serving agricultural workers and underserved populations.
  • High level of self‑motivation and ability to work independently and collaboratively.
  • Flexibility, adaptability, and a positive attitude.
  • Excellent interpersonal and diplomacy skills.
  • Strong problem‑solving and strategic‑thinking abilities.
  • Excellent written and verbal communication skills, including public speaking and presentations.
  • Ability to manage multiple tasks and meet deadlines without compromising quality.
  • Strong computer skills and proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
  • Valid California Driver’s License required.

Responsibilities

  • Proficient in multiple software and database systems including Salesforce and MS Office Suite of products.

  • Develop, implement, and executes CCOLE’s community health and outreach strategy for specific communities throughout Napa, Solano, and Yolo Counties, ensuring alignment with organization’s mission and goals and community needs.
  • Sets direction for annual calendar of community health and outreach events and activities tied to addressing the health and wellbeing needs of vulnerable populations in Napa, Solano, and Yolo Counties; leads collaboration with other departments or programs to align with the overall goals of the organization
  • Forge, maintain, and establishes excellent relationships with community partners, key stakeholders and elected officials; providing regular status updates and recommendations related to partnership opportunities; attend tri-county specific events, and work collaboratively with leadership to attend these events
  • Takes a lead role in the planning, execution, and evaluation of community health events and programs designed to increase organization visibility and promote community involvement. Examples include Dia de Muertos events, PRIDE events, Dia de la Familia, health screenings, and others
  • Collaborates with marketing department to promote CCOLE’s community health, outreach and education initiatives both internally and externally
  • Identifies mobile health partnerships and collaborates with clinical leadership to deploy health education campaigns and mobile health services to agriculture workers, students, unhoused residents, senior citizens and other vulnerable populations
  • Oversees culturally relevant activities and events that increases awareness of CCOLE services and engages community members in being more actively involved with their personal health, including Promotoras services
  • Oversees a robust community volunteer corp; Implement best-practices in volunteer management including use of technology systems for scheduling and tracking volunteer activity
  • Negotiate and manage contracts with vendors and service providers for community events and programs, ensuring cost-effectiveness and alignment with agency goals and objectives
  • Develop and monitor community health and outreach department budget, including grant deliverables.
  • Initiates collaborative working relationships with internal departments including Health Center Operations, Clinical, Dental, Pharmacy, Behavioral Health, Patient Services and others to identify community and patient population needs.
