POSITION SUMMARY This position provides managerial and clinical support to patients and staff. Uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. Demonstrates care coordination expertise and oversees the leadership of clinical, financial, and personnel management of the Care Coordination department. This position supervises employees and participates in selection, orientation, counseling, evaluation and staff scheduling. Maintains clinical, leadership and post-acute care services knowledge and competency to evaluate outcomes related to delivery of Care Coordination services.

Requirements

  • RN: Requires a Bachelor’s of Science in Nursing or in a related field. Requires a current Registered Nurse (R.N.) license in state worked.
  • Social Worker: Requires a Master’s degree in Social Work. Requires Licensed Master Social Worker (LMSW) (equivalent) or Licensed Clinical Social Worker (LCSW). An equivalent license applies to states that do not recognize an LMSW; therefore, the employee must possess a Master’s Degree and be a Licensed Social Worker.
  • For assignments in an acute care setting, Basic Life Support (BLS) certification is also required.
  • Required minimum of two years’ experience providing care in an acute patient setting.
  • Must possess a strong knowledge and understanding of care coordination as normally demonstrated through two years of related acute care patient management experience.
  • Demonstrates knowledge and skills of specialty area.
  • Requires critical thinking, communication, influence, decision-making, analytical, and flexibility skills to make optimal decisions based on multiple variables and desired outcomes.
  • Must have the ability to practice skills according to department and professional standards and quality requirements.

Nice To Haves

  • CCM (Certified Case Mgr) or other advanced case management certification.
  • Additional related education and/or experience preferred.

Responsibilities

  • Oversees Care Coordination personnel as assigned. Provides and/or facilitates Care Coordination and/or other related activities and serves as a resource to other staff members. Promotes utilization management through interdisciplinary patient care and activation of the discharge plan per the care delivery model.
  • Directs personnel actions including recruiting, new hire actions, interviewing and selection of new staff, salary determinations, training, and personnel evaluations. This position also participates in the development of Care Coordination goals and objectives in accordance with company standards. Ensures all goals and objectives are met timely and effectively. Hires, trains, conducts performance evaluations, and directs the workflow for the staff. This position is also accountable for participating in the development and implementation of department goals and objectives
  • Serves as a resource and provides leadership assistance to achieve optimal clinical, operational, financial, and satisfaction outcomes.
  • Supports and facilitates innovation and change management. Participates in the development, implementation and evaluation of the goals/objectives and process improvement activities within the department and organization. Analyzes data and healthcare trends to gain efficiencies and improve patient outcomes.
  • Participates in the development of the department budget in conjunction with established goals and objectives. This position is accountable for meeting annual budgetary goals.
  • Facilitates communication between members of the healthcare team to promote coordination of care with patients and families. Oversees facility patient satisfaction results specific to care activities and coaches and manages performance improvement activities to maintain positive patient satisfaction results.
  • Assesses patient satisfaction in areas of responsibility; sets a high standard for staff and leadership to improve patient satisfaction as measured by survey scores.
  • Responsible to ensure orientation with the expertise and knowledge base for child/adult protective services, public fiduciary/guardianship, and deeper understanding for the socioeconomic population for homeless, abuse, neglect, and placement/transitional care options creating the discharge plan.
  • This position is facility/entity based, providing leadership within transitional care coordination to ensure quality outcomes and resource efficiency. Internal customers: All levels of nursing leadership and staff and other members of the integrated health care team, for the purpose of integrating services, improving patient care, ensuring effective communication systems and facilitating decision making in clinical practice and business-related issues. External customers: Patients and families regarding patient care issues; physicians regarding patient care and program development; agency vendors and contracted services; staff from other health care agencies/providers and community/professional organizations for the purpose of exchanging patient and program information and insurance payers; and the Joint Commission, state, federal and community agencies regarding compliance with laws and regulations.
