About The Position

Under the direct supervision of the Manager of Case Management, the Associate Case Manager will maintain a caseload, providing coordinated and customized care plans for at-risk elderly (60 and over) seniors within the Orange County area. The Associate Case Manager will provide administrative and outreach support to the Case Management team.

Requirements

  • Associate of Arts Degree in Human Services; commensurate work experience may be considered in lieu of formal education.
  • Preference will be given to those with experience in the field of aging services.
  • Sensitivity to and awareness of the unique needs and resources available to this population.
  • Ability to maintain professional and confidential standards in client business-related activities.
  • Must be able to communicate clearly and effectively in person, over the phone, and via written communication.
  • Must have a valid California Driver’s license, automobile insurance, and be able to provide own transportation.
  • Must pass post-offer background check.
  • Bilingual preferred – Spanish or Vietnamese.

Responsibilities

  • Conducts client-specific assessment of needs, identifies problems, and establishes client-centered immediate needs and long-term goals.
  • Provides information and referrals for supportive services and entitlements consistent with mutually developed care plans.
  • Reassesses ongoing client needs and eligibility every three months and adjusts services as appropriate.
  • Consults with the Case Management team to problem-solve client concerns and identify appropriate interventions.
  • Maintains required records and reports in compliance with department, agency, local, state, and federal requirements.
  • Participates in agency meetings, team meetings, and training.
  • Supports the Case Management team with administrative tasks as needed.
  • Assists in developing and maintaining a current directory of community resources containing older adult human service programs and agencies in Orange County.
  • Assists HDM coordinators in troubleshooting failed/missed meal deliveries.
  • Monitors online inquiries and ensures connection to appropriate resources.
  • Serves as the first point of coverage for Company-staffed HDM sites as needed.
  • Participates in community outreach to inform and educate the general community about the home-delivered meals program as well as other agency programs.
  • Performs other duties as assigned relevant to Department functions.
  • Utilizes personal transportation to drive to client homes, partner agencies, and community sites.
