Requirements

  • Active New York State LPN License is MANDATORY.
  • Minimum of two (2) or more years of experience in senior care is REQUIRED.
  • Proven ability to supervise and lead a diverse team of caregivers.
  • Strong organizational skills with a high level of proficiency in medical recordkeeping.
  • Professional, compassionate, and resident-centered approach.
  • Excellent verbal and written communication skills for interacting with families and medical professionals.
  • Ability to remain calm and decisive in a fast-paced clinical environment.

Nice To Haves

  • Previous experience in Assisted Living, Adult Care, Home Care, or Long-Term Care is highly preferred.

Responsibilities

  • Oversee daily resident care services, ensuring the seamless execution of individualized care plans within the assisted living program.
  • Conduct comprehensive resident assessments and proactively monitor changes in clinical condition to ensure timely medical interventions.
  • Provide direct leadership, support, and professional guidance to the caregiving team, fostering a culture of accountability and empathy.
  • Maintain 100% compliance with New York State Assisted Living regulations and internal facility policies.
  • Serve as the primary point of contact for residents, families, and physicians; facilitate interdisciplinary team meetings to optimize care delivery.
  • Manage the clinical documentation and coordination for resident admissions, transfers, and discharge processes.
  • Address care-related issues in real-time and spearhead initiatives aimed at improving clinical outcomes and resident satisfaction.
  • Ensure all clinical, regulatory, and resident records are accurate, up-to-date, and audit-ready.
