About The Position

Position: Assistant Team Leader, Flexible ACT Function: Serves as a clinical staff member who supports the Flexible ACT Team Leader in the direction and supervision of staff activities, program operations, delivery of care and adherence with state and program regulatory requirements. Reports to: Flexible ACT Team Leader Location: The Bronx Schedule: Mondays through Fridays (9:00am-5:00pm) Tasks: Ensures the delivery of Flexible ACT program services in compliance with OMH and Medicaid regulatory standards. Supports the recruitment, training and supervision of program staff. Manages admissions and discharges, outreach and engagement, assessment and care coordination assignments of clients assigned ACT-Lite status within the Flexible ACT team. Facilitates bi-weekly client review meetings for those clients with ACT-Lite status. Ensures timely documentation of progress notes, assessments and care plans. Responds to inpatient/ER admission notification and contacts medical providers/hospitals for admission/discharge information. Participates in the quarterly case record review process. Ensures the appropriate use of client service dollars by approving plans and expenditures. Liaises with other providers to facilitate access to services and the coordination of care. Investigates incidents and reportable events in a timely fashion. Provides on-call emergency support. Other duties as assigned by supervisor. Requirements M.A. degree or higher in social work, psychology, rehabilitation counseling or a related field OR licensure/registration as a registered nurse. A minimum of 3-5 years of experience in the behavioral health field, preferably with experience on a multi-disciplinary mobile team. Demonstrated competency in clinical documentation, stage-wise treatment and motivational interviewing. Previous management and supervisory experience preferred. Strong written and verbal communication skills and strong leadership skills. Requirements
