About The Position

A respected and locally owned home health care agency in Indianapolis is seeking a compassionate and experienced Assistant RN Clinical Manager to support its growing patient base. This is a full-time, direct hire opportunity for a licensed RN with strong clinical assessment skills and a passion for home-based care. Reporting directly to the Clinical Director, this leadership position plays a critical role in overseeing nursing operations, ensuring regulatory compliance, and supporting interdisciplinary care across Medicaid-governed programs.

Requirements

  • Active RN License (State of Indiana)
  • Minimum 2 years of clinical nursing experience
  • Knowledge of CMS rules, Medicaid compliance, and home health regulations
  • Strong clinical assessment, time management, and interpersonal skills
  • Ability to supervise support staff (LPNs, CNAs, HHAs)

Nice To Haves

  • Bachelor's Degree in Nursing (BSN)
  • 1+ year of supervisory or case management experience
  • Skilled Home Health or Long-Term Care experience
  • Familiarity with OASIS documentation
  • Current CPR and BLS certifications
  • Exposure to CHAP, ACHC, or Joint Commission standards

Responsibilities

  • Serve as a clinical leader overseeing nursing care delivery in compliance with CMS Medicaid regulations and agency standards
  • Conduct in-home patient assessments and chart audits using OASIS
  • Ensure documentation accuracy to support timely billing
  • Supervise and support a team of LPNs, CNAs, and Home Health Aides (20-30 active cases)
  • Monitor clinical quality indicators; participate in monthly departmental reviews
  • Provide training, real-time corrections, and performance feedback to nursing staff
  • Act as the Alternate Clinical Supervisor with the State of Indiana
  • Partner with the Clinical Director on staffing, KPIs, scheduling, and patient care strategies
  • Maintain HIPAA compliance and uphold ethical standards in home health care
