The Phoenix at Milton is seeking an Assistant Program and Outreach Director (Activities Assistant) to join their team! The Assistant Program and Outreach Director reports directly to the Program and Outreach Director. This role is responsible for assisting in the day-to-day delivery of resident-centered activities that promote and integrate the Phoenix Senior Living philosophy, incorporating wellness in all aspects of the seniors' lives we serve. The position ensures that all dimensions (emotional, sense of purpose, physical, social, intellectual, and spiritual) are incorporated into the services provided and embraced.

  • Refer to the Resident Profile and addendum for every new resident.
  • Assist with facilitating a variety of appropriate and personalized activities that incorporate all dimensions of wellness and are available throughout the day and evening with resident care associate participation.
  • Understand the recognition of resident changes in condition and take appropriate action.
  • Assist residents to and from activity rooms when necessary.
  • In memory care, assist residents to and from Life Skills and other normalizing life-enriching activities and routines, and assist with engagement.
  • Review, read, notate, and initialize communication logs to document and learn about pertinent information and any resident's physical and behavioral changes.
  • Maintain activity areas in an orderly manner.
  • Assist with the delegation of daily care of any animals and/or plants within the activities program and services, as well as maintenance of records related to this care.
  • Assist in maintaining an inventory of activity and programming supplies, games, programs, and crafts.
  • High School Diploma, and job-related experience preferred in a senior living community.
  • Ability to lead activities and motivate volunteers and fellow associates for their involvement in social events and various scheduled activities.
  • Knowledge and experience in Assisted Living industry and Dementia care preferred.
  • Demonstrated ability to schedule, orientate, and train staff.
  • Understanding of infection control procedures.
  • Demonstrate the ability to plan and organize.
  • Demonstrate the ability to multi-task and manage stress.
  • Proficient in using MS Office.
  • Understand and assist with the development and implementation of Personalized Service Plans.
  • Understand advanced directives and end-of-life care.
  • Understand and embrace the assisted living philosophy.
  • Demonstrated knowledge of state regulations.
  • Proficient in using state audit compliance tools.
  • Proficient in time management skills and adherence to deadlines.
  • Ability to work weekends, evenings, and flexible hours.
  • Maintain appropriate driver's license such as CDL (commercial driver's license) or Chauffer license with an acceptable driving record in accordance with Federal Department of Transportation.
  • Exceptional grammar and documentation skills.
  • Understand basic concepts of risk management.
