This faculty member will practice as a member of multidisciplinary teams at the University of Colorado Family Medicine (50%) and Internal Medicine clinics (30%), with other academic responsibilities (20%) in teaching, scholarship, and service. These clinics are located in the Denver/Aurora metro regions of Colorado. The faculty member will collaborate to provide clinical coverage with other faculty colleague team members in internal medicine and family medicine clinics. This faculty member will provide exemplary care to patients and families, further develop innovative clinical services for patients and providers, and serve as a practice and training site. This position will provide clinical pharmacy services as a member of the interdisciplinary primary care team. Primary clinical responsibilities include developing and providing direct patient care clinical pharmacy services that focus on chronic disease management using collaborative drug therapy management protocols. Successful candidates will also interface with clinic providers to provide comprehensive medication management and other population health initiatives for patients. Clinical services and provider support will be delivered through a hybrid model that includes both in-person and telehealth/remote interactions with patients and providers. In addition to clinical practice, successful candidates will be expected to provide experiential teaching for students in the Doctor of Pharmacy curriculum. The faculty member is expected to engage in scholarship that supports and advances their clinical and educational activities.
