About The Position

The Assistant Manager of Continuing Care Operations at Kaiser Permanente is responsible for overseeing daily operations in the NHS care areas, ensuring optimal delivery of patient care in sub-acute and long-term care environments. This role involves managing Case Management teams, collaborating with NHS providers, and supporting operational needs to enhance patient experience and clinical quality standards.

Requirements

  • Minimum two (2) years of Case Management experience as a nurse in a subacute environment or hospital.
  • Minimum one (1) year of experience as a charge nurse, lead, or supervisor in a clinical environment.
  • Bachelor's degree in nursing.
  • Registered Nurse License (Washington) required at hire OR Compact License.

Nice To Haves

  • Two (2) years of experience as clinical lead or supervisor in a long-term care environment.
  • Three (3) years of increasingly responsible operations experience as a supervisor or lead case manager.
  • Knowledge of regulatory requirements, managed care, and health care delivery systems.

Responsibilities

  • Manage daily operations of assigned members requiring sub-acute and long-term care within the continuing care department.
  • Support coordination of workflow across the care continuum in partnership with NHS leadership.
  • Collaborate with managers and CMLNs to align operations and implement interdisciplinary change initiatives.
  • Ensure implementation of IQM and CCQM quality initiatives to support optimal care.
  • Assure adequate staffing levels to meet patient needs in the post-acute environment.
  • Train and support staff in delivering a consistent customer experience and resolve service issues.
  • Manage staff selection, training, performance management, and engagement.
  • Assist with managing budgets for care management and identify innovative performance solutions.
  • Establish processes to monitor compliance with operational standards and regulations.
