Responsible for the overall leadership of the case management department with a primary focus on Intake and Enrollment. Responsibilities include hiring, training and supervising Case Manager(s). The Assistant Director is responsible for ensuring the highest level of performance of all staff on all shifts and that the clients have excellent care based on the SVDP model of Trauma Informed Care, Crisis Prevention and Mediation. The Assistant Director oversees coordination of all aspects of veterans’ transition into emergency housing and from emergency housing to permanent housing. This position acts in the capacity of the Director, Veteran Housing Services capacity in his/her absence. Primary duties include hiring, orienting, training, supervising and evaluating the Case Manager(s) assisting in the development of a detailed Individual Service Plan (ISP) for each client, in collaboration with the Housing Coordinators. Ensuring that all staff adheres to the principles of Trauma Informed Care, Crisis Prevention and Mediation. Developing and providing a program of ongoing training for staff. Conducting regular team meetings, including all-staff meetings, facilities teams meeting, and individual supervision meetings. Developing programs for staff incentives and retentions to maintain a positive workplace morale. Developing and implementing a consistent system of rewards and consequences for clients and ensuring that they are monitored and evaluated in a timely fashion. Ensuring the appropriate documentation is kept and read on veterans including, but not limited to, case notes, incident reports, and housing stability plans. Collecting monthly statistics and submitting them to Program Director on a monthly basis. Reviewing HMIS weekly to ensure all HMIS data is entered in a timely manner. Conducting monthly review of active client case files to ensure all intake documents are updated and included in each case file. Ensuring the appropriate documentation is reviewed and retained including, but not limited to, intake/discharge, incident reports, and daily logs. Discharging families transitioning from the Veteran Housing Services program and ensuring that all discharge paperwork has been completed by case manager prior to the Intake Discharge Rapid Resolution Specialist exiting them from HMIS. Responding in a trauma-informed manner to health and mental health situations that arise to ensure that safety of all is maintained and that appropriate resources are summoned as needed. Ensuring staff enter relevant information into Salesforce within 24 hours. Secondary duties include supervising the Intake/Discharge/Rapid Resolution Specialist. Keeping updated on the most effective means of accessing services available within the larger community and maintaining an active database for referrals and community partners. Attending and fully participating in all necessary staff meetings, other internal and external meetings, as well as meetings with on and off-site service providers. Creating an atmosphere of care and respect for clients. Attending meetings and events related to St. Vincent de Paul and the Center. Upholding the vision, mission, and values of St. Vincent de Paul. Other duties as assigned, within the scope of the position.
