About The Position

Visiting Nurse Association of Ohio is building something rare — a community-based behavioral health program inside one of the state's most established home health organizations — and they need a clinical leader who can run with it. Requirements

• Active, unencumbered RN license in Ohio
• BSN required; MSN or related master's preferred
• 3–5 years of behavioral health clinical experience, including mental health and co-occurring conditions
• 2+ years of leadership or supervisory experience
• Knowledge of Medicare home health regulations, behavioral health standards, and payer requirements

Nice To Haves

• Hospital, ED, inpatient psych, or acute behavioral health experience strongly preferred
• Familiarity with PDGM, CHAP, CARF, and Medicaid documentation standards a plus

Responsibilities

• Lead clinical operations for VNA of Ohio's community-based behavioral health home health program
• Drive census growth by building referral partnerships with hospitals, EDs, physician practices, and community organizations
• Ensure compliance across CMS, CHAP, CARF, Medicaid, and state requirements
• Oversee quality metrics across dual frameworks — OASIS and Medicaid mental health measures
• Recruit, develop, and retain behavioral health nurses, psychiatric providers, therapists, and care coordination staff
• Support crisis response and escalation for patients experiencing behavioral health decompensation
• Partner with the Medical Director and interdisciplinary teams to deliver integrated, patient-centered care
