Overview Under the direction of the Director of Nursing, the Assistant Director of Nursing is responsible for providing quality care to all residents of the Caleb Hitchcock Health Center. This position is expected to be consistently present on the nursing units (“on the floor”), actively participating in resident care, staff support, and real-time clinical decision-making. The Assistant Director of Nursing functions as a professional registered nurse who is accountable for assessing, planning, implementing, and evaluating all aspects of resident care, including physical, psychosocial, and environmental needs. Administrative responsibilities support, but do not replace, the primary clinical focus of this role.

Requirements

  • Bachelor’s degree in Nursing preferred.
  • Current CT RN license is required.
  • 2 years of full-time clinical experience in nursing is required.
  • 2 years of supervisory experience is preferred.
  • Geriatric experience is required.
  • Must be able to speak, read, and write the English language in an understandable manner.
  • Must have a strong working knowledge of Microsoft Office applications (e.g., Word, Excel, Outlook, PowerPoint).
  • Comprehensive knowledge of geriatric nursing care, principles of nursing, and physical assessment skills.
  • General knowledge of supervisory principles and practices.
  • Basic knowledge of human relations skills necessary to deal effectively with geriatric residents, their families, and CHHC staff.
  • Ability to focus on task needs and perform intellectual executive functions as related to documentation, charting, care device operations, etc.
  • Ability to execute intellectual tasks and duties under conditions of high demand and distraction.
  • Ability to complete the essential functions of the position with or with adaptive personal devices (e.g., hearing aid, corrective lenses).
  • Ability to operate equipment and technology required for the position.
  • Able to wear personal protective equipment.
  • Must be able to communicate effectively.
  • Able to assist with the evacuation of residents under emergency status to the best of their physical ability.
  • Able to function independently with integrity and work effectively with residents, personnel, and support agencies.
  • Able to constantly stand or walk to provide resident care and assistance.
  • Able to frequently push and/or pull a force up to 30 pounds including residents weighing up to 250 pounds in wheelchairs and geri-chairs.
  • Able to reach overhead, bend, and carry or lift 50 pounds occasionally and 35 pounds frequently.
  • Able to frequently transfer and support residents weighing up to 250 pounds while positioning, and transferring residents to beds, toilets wheelchairs, etc. Additional employees or mechanical assistance may be used.
  • Able to frequently reposition residents weighing up to 250 pounds in bed for assistance with treatments, hygiene, and comfort needs. Additional employees or mechanical assistance may be used.

Responsibilities

  • In conjunction with the Director of Nursing, has complete oversight of nursing care for all residents.
  • In the absence of the Director of Nursing, acts as the Director of Nursing.
  • Provides direct, hands-on nursing care to residents on a routine basis.
  • Maintains a visible and consistent presence on the nursing units throughout scheduled shifts.
  • Assesses resident’s conditions and responds promptly to changes in status.
  • Administers medications accurately and in a timely fashion.
  • Utilizes electronic medical records (EMR) system and documents according to nursing practice standards and Duncaster policies and procedures.
  • Provides treatments to residents in a timely fashion according to physician orders and Duncaster policies and procedures.
  • Provides physical comfort and a safe environment.
  • Initiates, implements and evaluates care plans/MDS on an ongoing basis.
  • Accurately completes admission and discharge procedures.
  • Coordinates nursing staff and resident care assignments to ensure optimal clinical coverage
  • Serves as a direct point of communication between patients, families, nursing staff, and physicians.
  • Communicates clearly and compassionately with patients and their family members and offers emotional support when in distress.
  • Follows all policies and procedures, annual mandatory in-services, required meetings, and other conditions of employment.
  • Review updates and communicate with staff, provides real time training of staff and providers, unlock/reset passwords, troubleshoot issues and work with the EMR system to resolve.
  • Works with liaison to ensure active list of users, register new users, train new employees, update inventory to meet facility needs.
  • Performs bi-monthly pharmacy audits, narcotic destruction, pharmacy returns and pharmacy expense monitoring. Maintains a relationship with pharmacy to optimize processes/costs.
  • Collaborates with interdisciplinary team members to support quality outcomes. Participates in QAPI program including audits, data collection and analysis and process improvement.
  • Monitors staff attendance, track patterns and identify, supports onboarding, mentors and coached new staff and identifies training needs through direct observation and interaction.
  • Assigns and monitors nursing call schedules.
  • Performs other duties as assigned.
