About The Position

The Assistant Director of Health & Wellness at Discovery Senior Living is responsible for leading and coordinating the health and well-being of residents within the community. This role involves developing and implementing staff schedules, ensuring compliance with healthcare regulations, and maintaining communication with residents and their families regarding health concerns. The position requires collaboration with the Director of Health & Wellness and other team members to assess and address the ongoing needs of residents, promoting their dignity and independence.

Requirements

  • Maintains current state license as a Licensed Professional Nurse or a Professional Registered Nurse per state regulations.
  • Experience in assisted living, home health, or long-term care industries.
  • Two (2) years experience as a Licensed Nurse preferred.
  • One (1) year nursing management experience preferred.
  • Demonstrates competence in assessment skills, injections/medication administration, follow-up, and triage.

Responsibilities

  • Works with the Director of Health & Wellness (DHW) and assists with assigned duties.
  • Creates and posts a timely schedule for all personal care staff.
  • Responsible for filling any call-offs while ensuring proper staffing levels.
  • Steps in to fill a staff vacancy if unable to fill through other staff members.
  • Ensures compliance with federal, state, and local laws and regulations regarding clinical care.
  • Partners with DHW and team members to analyze and maintain resident and team member health safety.
  • Identifies ongoing needs and services of residents through assessments and service plans in collaboration with DHW.
  • Maintains communication with residents' families regarding changes in care or health concerns.
  • Reviews service plans to learn pertinent information about residents.
  • Assists/observes medications and treatments for each resident in accordance with state regulations.
  • Maintains confidentiality of all resident information, including medication.
  • Helps residents maintain independence and promotes their dignity and physical safety.
  • Strives to understand and respond to each resident with empathy, considering their unique communication patterns and needs.
  • Practices good standard care precautions of cleanliness, hygiene, and health standards.
  • Notifies the Director of Health Care of any resident and/or family concerns.
  • Actively participates in change of shift reporting of pertinent information.
  • Participates in the development of the Service Plan and monthly updates.
  • Addresses all accidents/incidents immediately and completes incident reports.
  • Addresses unsafe and hazardous conditions/equipment immediately.
  • Addresses occupational exposures to blood, body fluids, and hazardous materials immediately.
