About The Position
In this role, the Assistant Care Manager (ACM) is responsible for completing daily or shift specific quality audits, facilitating monthly team meetings, and directing delivery of quality, patient-centered care for individuals seeking linkage to care and supportive services, particularly therapeutic services. This is in collaboration with and under the supervision of the Director of the Population Centered Health Homes (PCHH). In the absence of the team's Director, the ACM assumes responsibility for the Supportive Services unit. Availability to work weekends and evenings as needed.
