About The Position

The Assessor will work with the Client Services team at Interfaith Ministries for Greater Houston, responsible for conducting initial assessments and reassessments of clients, particularly focusing on the Meals on Wheels program. This role is crucial for maintaining client documentation and ensuring the safety and well-being of clients receiving services.

Requirements

  • Bachelor's Degree in Social Work preferred.
  • Bilingual in Spanish desirable.
  • One year of client case work experience preferred.
  • Experience working with low-income elderly individuals is a plus.
  • Excellent interpersonal skills with effective verbal and written communication abilities.
  • Ability to prioritize tasks and manage time effectively.
  • Skilled in working independently under pressure while maintaining accuracy.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook).
  • Valid driver's license and reliable automobile with insurance required.

Nice To Haves

  • Experience in senior care is a plus.
  • Enjoy working with senior citizens.

Responsibilities

  • Conduct home/telephone visits to assess clients' ability to consume and store meals and ensure staff safety during meal delivery.
  • Perform thorough and accurate client assessments and reassessments during visits.
  • Ensure electronic documentation of assessments is organized for easy retrieval.
  • Determine eligibility of potential clients for nutrition services.
  • Provide nutritional information and referrals to clients, collaborating with the Client Services Team.
  • Report any client abuse or neglect to state authorities and internal contacts.
  • Complete all assigned assessments and reassessments in a timely manner.
  • Assist with large filing projects and perform other duties as assigned by the supervisor.
  • Submit timely weekly and monthly productivity reports to the Data/Billing department and Manager of Client Services.
  • Attend weekly team meetings and report service issues or complaints to appropriate departments.
