About The Position

The Assessment, Registered Nurse (ARN) is responsible for assessing prospective enrollees and active health plan members to determine potential care needs and plan eligibility. This position involves conducting virtual or in-person visits with potential enrollees and active members of the health plan as assigned, providing a comprehensive overview of program benefits to applicants, their family members, and caregivers. The ARN educates consumers and members on their rights, responsibilities, and the services offered by Nascentia Health Options, and interviews members, family members, or significant others identified through case findings to assess their service needs. The role also includes reviewing the member handbook, provider directory, and covered services during the visit, evaluating previous assessment information, and performing all assessment activities in compliance with regulatory agency requirements. The ARN assists in identifying unmet needs and creating person-centered service plans to address them, communicates effectively with potential members, their families, care management staff, and providers, and collaborates with the healthcare team to address potential enrollees' needs. Additionally, the ARN gathers information on eligibility, benefit coverage, and benefit coordination, ensures accurate completion of all required documentation in the health plan's electronic medical record (EMR) and quality review programs, participates in departmental meetings, and adheres to all state and federal privacy regulations, including HIPAA. The ARN acts as a community liaison for the health plan and performs other duties as assigned.

Requirements

  • Possession of a current Registered Professional Nurse license in New York State is required.
  • A Bachelor of Science in Nursing (BSN) is preferred, while an associate degree is mandatory.
  • Previous nursing experience of one year is preferred.
  • Previous experience in long-term care and/or managed care settings is a plus.
  • Knowledge of homecare, Medicare, and Medicaid is beneficial.
  • Bilingual skills are a plus.
  • Demonstrated ability to work independently and effectively manage workload priorities.
  • Proficient in computer usage with a working knowledge of basic computer skills and Microsoft Office applications.
  • Excellent interpersonal, virtual, and telephonic communication abilities.
  • Valid New York State drivers license and ability to drive in all types of weather.
  • Aptitude to comprehend and utilize the organization's software systems to execute job responsibilities.

  • Adheres to all state and federal privacy regulations, including HIPAA, and Nascentia Health Options policies regarding confidentiality, privacy, and security.
  • Maintains accurate scheduling information.
  • Acts as a community liaison for the health plan.
  • Other duties as assigned.
