Our professional nurses play a crucial role in planning and directing services to meet the needs of individuals and families within their homes and communities. As a Homecare Assessment Nurse, you will work independently, exercising good judgment and initiative while adhering to company policies. You will conduct regular visits to clients' homes, providing vital assessments and building strong, positive first impressions. The role requires attention to detail, the ability to remain calm under pressure, and effective communication.

  • Contact clients to set up visit appointments.
  • Introduce yourself and provide essential materials about AnswerCare.
  • Conduct initial and ongoing site assessments, including personal information, home condition, and safety features.
  • Gather detailed information on the client's medical history, mental and behavior conditions, routines, habits, and preferences to create a comprehensive care plan.
  • Make necessary revisions to the care plan as patient status and needs change.
  • Maintain open communication with nursing shifts, schedulers, supervisors, families, and case managers.
  • Conduct assessments every 60 days, combined with Satisfaction Surveys.
  • Record vital signs during visits and note any significant changes.
  • Conduct unplanned assessments in case of emergencies or as requested.
  • Monitor client condition and supervise caregivers to ensure adherence to the care plan.
  • Advocate for clients to ensure their rights are met and maintain client confidentiality.
  • Effectively communicate relevant information to enhance the care plan.
  • Prepare clinical notes and update the RN regularly, at least every 60 days.
  • Coordinate with Case Managers to adjust the care plan as needed.
  • Current certificate in standard first aid and CPR.
  • Current LPN or RN license in this state.
  • Valid driver's license and reliable transportation.
  • Must be 18 years of age or older.
  • Clean criminal history.
  • Must have State ID.
  • Eligibility to work in the U.S.
  • Current listing on Nurse Registry.
  • Punctuality and reliability.
  • Professional appearance and demeanor; wear your AnswerCare name badge during visits.
  • Be open, kind, empathetic, courteous, and attentive.
