About The Position

We are looking for an amazing Assessment Nurse Specialist who promote the mission and core values of the Community and LCB Senior Living to residents, visitors, co-workers and the surrounding community. The Nursing Assessment Specialist is designed to enhance the quality, consistency, and timeliness of outside community assessments for prospective residents. This role will serve as a key clinical partner to the Sales and Community team, ensuring accurate assessments, streamlined move-in preparation, and strong clinical onboarding for new residents. This position will conduct and prioritize external community assessments across Massachusetts, Rhode Island, New Hampshire, and Connecticut based on assessment volume and leasing velocity. The Specialist will complete all required documentation in Yardi, support care planning for move-ins, and collaborate with communities to ensure smooth transitions

  • Minimum of 3-5 years of experience in an assisted living or healthcare or similar role.
  • Microsoft Suite, Yardi, and other EHR as needed

Responsibilities

  • Conduct in person, Yardi and LCB comprehensive nursing assessments for prospective residents in MA, RI, NH.
  • Prioritize and manage assessment scheduling based on referral volume, market demands, and leasing velocity for assigned communities.
  • Ensure timely completion of assessments and post visit communication of findings to Sales, Operations, and community teams.
  • Serve as a clinical liaison to the Sales team to ensure timely scheduling, completion, and follow-up of assessments.
  • Participate in assessment debrief discussions, including Diamond Solutions calls, as requested.
  • Holds high standards of clinical excellence.
  • Ensure all clinical work is accurate, complete, and available to receiving communities to support successful move in process.
  • Enters all assessment information into Yardi in accordance with LCB documentation standards and timelines.
  • Maintain effective, professional communication with Sales Directors, Executive Directors, Resident Care Directors, and other stakeholders.
  • Provide timely clinical insights and recommendations based on assessment findings.
  • Support cross functional efforts to improve assessment processes and resident onboarding.
  • Ensures timely clinical record documentation for accuracy, timeliness and applicability to LCB care standards.
  • Provide education and learning on topics to support operations, clinical, sales and successful move in process.
  • Maintain current professional knowledge by attending continuing education programs .
  • Assist in the implementation of clinical initiatives, training, and continuous improvement projects.
  • Frequent travel anticipated throughout MA, RI, NH and CT to complete assessments.
  • May represent the company in a public-contact role (i.e. job fair, health care convention).
  • Accepts and performs other assignments as directed by the VP of Care.
  • May initiate projects/procedures to accomplish agreed upon objectives.
  • Functions independently with extensive latitude in organizing, scheduling, and work completion.
  • Provides direction/guidance for decisions regarding care assessment outcomes, documentation, consults with Community Executive Director, RCD, Sales and Marketing Director and Reflections Director regularly.
