About The Position

CMHOC is looking for a full-time Assessment and Level of Care Specialist. This position will work Monday through Friday, 8:00 am to 5:00 pm at 12265 James St, Holland, MI 49424. This is a professional position responsible for administering a variety of assessments to determine amount, scope and duration of eligible services. This position will assess medical necessity using both formal and informal assessment tools and provide recommended services to the consumers Supports Coordinator.

Requirements

• Master's degree in social work, psychology, or a related mental health field is required.
• Two years of relevant professional experience in a mental health agency preferred.
• Must possess or obtain State of Michigan licensure as a Licensed Master's Social Worker, Limited Licensed Master's Social Worker, Licensed Professional Counselor or Limited Licensed Psychologist.
• Must be a Qualified Intellectual Disability Professional (QIDP).
• Thorough working knowledge of DSM- V.
• Thorough working knowledge of structured assessment tools such as the Support Intensity Scale.
• Thorough working knowledge of the Michigan Mental Health Code.
• Thorough working knowledge of varied therapeutic interventions.
• Thorough working knowledge of local community resources.
• Good understanding of agency processes, procedures, and quality assurance principles.
• Excellent interpersonal communication skills.
• Excellent verbal and written communication skills.
• Participates with and accepts clinical and administrative supervision.
• Shows willingness to learn and use new skills, and takes regular advantage of internal and external training and staff development opportunities and resources.
• Thorough working knowledge of proper medical record documentation and management.

Responsibilities

• Provides quality direct consumer care encompassing evaluation and assessment, diagnosis, and treatment planning as appropriate.
• Creates a narrative psychosocial summary that moves from details collected to understanding individual preferences, formulates recommendations for level of care, intensity of supports and possible goals which can be shared with the individual and their Support Coordinator.
• Provides consultation on the development of the Individual Plan of Services that incorporates the recommendations of the psychosocial assessment.
• Aware of and uses uniform, standard, population specific assessment tools, relevant of professional expertise, psychosocial and functional assessment along with sound professional judgment to determine level of care including amount, scope and duration.
• Provides authorization and reauthorization for appropriate level of care and corresponding eligible services.
• Ensures that services to consumers meet CMH standards of quality and quantity.
• Provides consultation to other agencies regarding services and mental health issues of consumers and referrals.
• Collaborates effectively and efficiently with other community agencies.
• Maintains all agency paperwork and clinical record documentation in an accurate and timely fashion.
• Attends and participates in agency meetings as well as program specific team meetings.
• Engages in utilization management activities as needed.
