Build Your Purpose. Compensation: $25.02 - $28.40 Final compensation is determined by factors such as education, licensure level, experience and other qualifications. Position Description: Monitor and oversee all medical/clinical aspects of services delivered. Provide medication monitoring and documentation. Deliver psychopharmacologic and medical services within your scope of practice. Collaborate with ACT team members to monitor clinical status and response to treatment. Conduct crisis assessments, symptom management, and intervention planning. Provide individual counseling, substance abuse treatment, and psycho-educational support. Assist in creating transition plans when clients move in and out of services. Act as a consultant to primary healthcare providers and community medical staff. Ensure proper documentation, including treatment plans and progress notes. Be available for crisis response and support to the ACT team on a 24/7/365 basis

  • Active LPN license in North Carolina.
  • At least 1 year experience with population served.
  • Each ACT team member must complete DMH-approved ACTT training.
  • NVCI certification required.
  • Training in population served, client rights, and confidentiality.
  • Valid driver’s license and current automobile insurance required.
