About The Position

New Horizon Counseling Center is committed to empowering individuals living with serious mental illness to lead meaningful, independent lives. Through compassionate, community-based care, we provide comprehensive behavioral health services that meet clients where they are. We are seeking a motivated and experienced professional to serve as the ACT Assistant Team Leader. In this role, you will support the Team Leader in overseeing daily operations and clinical service delivery while also providing direct services to clients in the community. The Assistant Team Leader helps ensure program fidelity, staff support, and high-quality, coordinated care for individuals with complex behavioral health needs.

Requirements

  • Master’s degree in Social Work, Counseling, Psychology, or a related field preferred (Bachelor’s with significant experience considered)
  • Relevant licensure or certification preferred
  • Minimum of 2–5 years of experience working with individuals with serious mental illness
  • Knowledge of the ACT model, recovery-oriented practices, and community-based services
  • Strong leadership, organizational, and interpersonal skills
  • Ability to balance administrative responsibilities with direct service delivery

Nice To Haves

  • Experience working in ACT, community mental health, or integrated care settings
  • Bilingual skills are a plus
  • Familiarity with electronic health records (EHR) systems

Responsibilities

  • Assist the Team Leader in managing day-to-day operations of the ACT team
  • Provide supervision, guidance, and support to team members as delegated
  • Participate in staff training, coaching, and performance support activities
  • Help coordinate daily team meetings, case conferences, and service planning
  • Ensure the development and implementation of individualized, recovery-oriented treatment plans
  • Provide direct, community-based services and crisis intervention as needed
  • Monitor service delivery to ensure compliance with ACT model standards and agency policies
  • Collaborate with internal staff and external providers to ensure continuity of care
  • Support quality improvement initiatives and program evaluation efforts
  • Maintain accurate and timely documentation in accordance with regulatory requirements
  • Other relevant duties as assigned
