About The Position

Traditional Home Health Care is a leader in the health care field, providing skilled nursing/aide services and state-of-the-art educational programs. Their primary goals are to restore patients' health and increase their confidence in their own abilities, enabling them to lead healthier, more productive lives. The Art Therapist will be responsible for visiting patients, assessing their care needs, and collaborating with the patient, family, and physician to develop care plans. This role involves assisting patients with activities of daily living, fostering their efforts towards self-sufficiency and independence, and educating and supervising patients and their families in the home environment on treatment, self-care, coping strategies, and preventive health measures. Effective communication with all involved parties, including patients, families, agency staff, physicians, and other disciplines, is crucial for carrying out treatment plan objectives.

Requirements

Must have a current drivers license
Ability to travel to case assignments as assigned
Ability to work independently with minimal supervision and manage time effectively
Strong interpersonal skills
Current license as a Art Therapist in Pennsylvania

Responsibilities

Visit patient and assess patient, care and plans with patient/family and physician
Assist the patient with activities of daily living and facilitate their efforts towards self-sufficiency and independence
Teaches and supervises patient and family in the home environment regarding treatment, self-care, coping and preventive health measures
Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (i.e., social services, therapists, etc.) to carry out treatment plan objectives
