The APRN - Internal Medicine position at Yale New Haven Health involves conducting comprehensive health assessments, developing and implementing individualized treatment plans, prescribing medications, and providing patient education. The role requires collaboration with a healthcare team to ensure coordinated patient care and participation in quality improvement initiatives. The position is full-time, with a schedule of 40 hours per week, including 36 clinical and 4 administrative hours, and utilizes the Epic EMR system.

  • Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
  • Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
  • Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.
  • Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.
  • Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma).
  • Perform minor office procedures as trained and within scope of practice.
  • Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care.
  • Document all patient encounters accurately and thoroughly in the electronic health record.
  • Participate in quality improvement initiatives and adhere to all organizational policies and procedures.
  • Serve as a resource for medical staff and participate in educational activities as needed.
  • Provide age-appropriate care to patients across the lifespan, from infants to geriatrics.
  • Manage referrals to specialists and coordinate follow-up care.
  • Maintain current knowledge of family medicine best practices and engage in ongoing professional development.
  • 1-2 years clinical experience in a primary care/internal medicine setting.
  • Active PA or NP in family medicine specialty certification.
  • Active, unrestricted CT medical license or in process of obtaining.
  • ACLS/BLS/PALS (as required) Certification.
  • DEA License.
  • Prefer 1-2 years in a clinical working experience as a NP/PA within the given specialty/service line.
