About The Position

The Primary Care Office Advanced Practice Provider (APRN or PA) is responsible for providing total care for assigned patients in collaboration with the physician and patient care team. This role requires a license in good standing with the State of Florida. The APP will manage patient care across all providers, family, and caregivers, ensuring timely, patient-centered, evidence-based medicine for acute, chronic, and palliative care needs. Key duties include patient assessment/exam, care plan creation, intervention, diagnosing and treating patients, analyzing lab results and x-rays, writing prescriptions, taking patient histories, providing surgical assistance, and ordering home health, therapies, and DME. The APP will also instruct on symptom management and advanced care planning for chronic life-limiting illnesses, address Quality Measures and HCCs, and maintain ongoing communication with the Primary Care Provider, consulting with the supervising physician on complex decisions. The role may involve working with Palliative/Hospice Care agencies for end-of-life care and requires timely documentation and billing. Additionally, the APP will prevent unnecessary Emergency Department utilization, ensure accurate documentation in the electronic medical record per MPG policy and CMS guidelines, and demonstrate strong clinical and interpersonal skills for an aging adult population. The APP provides leadership in applying the nursing process, initiates interdisciplinary projects, identifies patients for skilled nursing/therapy, follows CMS guidelines for SNF admissions, and makes referrals to community resources. The APP also collaborates with the Program Manager in overseeing assigned Medical Assistants, establishing workflows, and providing training.

Responsibilities

  • Provide total care for all assigned patients in collaboration with the community-based physician/provider and patient care team.
  • Communicate and manage the patients care across all care providers, family, and caregivers.
  • Ensure that timely, patient-centered, appropriate evidence-based medicine is provided for patients with acute, chronic, and palliative care needs.
  • Perform a patient assessment/exam, create a care plan, and provide intervention as indicated.
  • Diagnose and decide treatment for patients.
  • Analyze and interpret x-rays and lab results.
  • Write prescriptions.
  • Take patients histories.
  • Provide surgical assistance.
  • Order home health, various therapies, and DME as indicated.
  • Provide instruction and assistance on symptom management and advanced care planning for patients with chronic life limiting illness.
  • Assist in addressing outstanding Quality Measures and HCCs as applicable to meet or exceed all quality goals.
  • Maintain ongoing communication with the Primary Care Provider to ensure continuity and coordination of care.
  • Consult with the supervising physician on any complicated diagnosis and treatment decisions.
  • Work with regional Palliative/Hospice Care agencies to facilitate end-of-life care and services when appropriate.
  • Complete all documentation and billing in a timely manner, per protocol.
  • Provide services to meet patient needs and prevent unnecessary Emergency Department utilization when appropriate.
  • Ensure all pertinent and active medical conditions are documented in the electronic medical record in a timely manner, consistent with Millennium Physician Group (MPG) policy, and in a compliant manner with Centers for Medicare & Medicaid Services (CMS) and other insurance carriers.
  • Exhibit strong clinical skills to improve patient care for an aging, adult population.
  • Exhibit excellent interpersonal relationship skills in interpreting and communicating health care information to the patient, family, and patient care team.
  • Provide leadership in the application of the nursing process to client care, organizational processes and/or systems improving outcomes at the program or service level.
  • Initiate interdisciplinary projects to improve organizational performance.
  • Identify patients who are appropriate and would benefit from skilled nursing/therapy and initiate appropriate process for qualifying value-based patients.
  • Follow all CMS guidelines and ensure patients are admitted to a qualifying SNF.
  • Complete all required documentation and work with other members of the health care team to ensure continuity and patient safety.
  • Instruct and counsel patients regarding compliance with prescribed therapeutic regimens and health maintenance.
  • Identify patients needing additional assistance and make referrals to home care, social services, behavioral health, and other available community resources, when needed and appropriate.
  • Work collaboratively with Program Manager in oversight of assigned Medical Assistant and to establish workflow processes, provisions of ongoing education/training, and employee improvement planning as needed.
