About The Position

The APRN/PA supports the clinical operation of the Transplant Institute with primary focus on clinical services in the inpatient and outpatient setting. Delivery of patient care is conducted in collaboration with the transplant surgeons and physicians including, but not limited to, physical exams, diagnosis and treatment, ordering and interpreting tests, counseling on preventative health, writing prescriptions and assisting in surgery. In addition, the APRN/PA is responsible for coordinating and managing the patient care of high-risk patient populations throughout the continuum to optimize the patient's clinical, functional and psychosocial status, to provide the communication link for coordination of care among the multidisciplinary healthcare team involved with this patient type, and to orchestrate a performance improvement program that focuses on improving patient outcomes. Under administrative guidance and physician supervision, provides comprehensive primary care services, acute care services and extended care services in the area of nursing and/or medicine for individuals and their families. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

Requirements

  • Bachelor's of Nursing or Master's of Nursing Required.
  • If Nurse Practitioner - must have successfully completed undergraduate nursing programs, passed state boards, and achieved ARNP status.
  • 3 Work Experience Required.
  • Basic Cardiac Life Support (BCLS) Required.
  • Advanced Cardiac Life Support Cert (ACLS) Required.
  • American Academy of Nurse Practitioners (AANP) National Nurse Practitioner certification Required.
  • Physician Assistant (PA) Required.
  • If Physician Assistant - must hold a baccalaureate degree, or a minimum of two years of college credits and graduated from a physician assistant program approved by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).
  • Advanced Practice Registered Nurse (APRN) Required.
  • APRN-BC - Advanced Practice Registered Nurse, Board Certified Required.
  • American Nurses Credentialing Center (ANCC) Required.

Responsibilities

  • Utilizes sensitivity, responsiveness, caring and compassionate attitude with patients, families and visitors by listening and communicating effectively with courtesy, consideration and tact.
  • Facilitates and coordinates evaluations for potential organ recipients or living donors.
  • Evaluates and treats patients with acute, chronic complaints and health maintenance concerns.
  • Attends and participates in transplant team meetings including Patient Review Committee, hospital interdisciplinary rounds, discharge planning and QAPI.
