About The Position
Provides expertise in caring for patients with life limiting, serious illness and their families. Responsible for synthesizing complex data to assess, diagnose, implement and coordinate plans of care that assist symptom control, functional status and enhanced communication. Overall goal is to improve quality of life for every patient and family we have the privilege of serving. This is a FT M-F day shift position with the hours 8:00am-530pm covering Ohio's Hospice South region to include the greater Dayton/Montgomery County and surrounding areas. This position will provide Community based palliative care and Hospice care and will have some on call rotations as well. We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed.
