About The Position

The APP Program Coordinator II for Sickle Cell at Summa Health Medical Group is responsible for collaborating with the Medical Director to provide support and direction to all staff within the Internal Medicine Center and across the hospital system. The role focuses on ensuring timely and quality care for patients in the Sickle Cell specialty program, working closely with departments such as Hematology and Pain Management. The coordinator will develop and advance the quality of advanced practice through specialized direct patient care, leadership, consultation, education, and research. The position also involves improving processes of care for patients and maintaining clinical competency through a clinical caseload providing primary and specialty care to the identified Sickle Cell patient population.

Requirements

  • Graduate of a nationally accredited Physician Assistant program or Master's Degree in Nursing with completion of Advanced Practice program.
  • Current license to practice as an advanced practice registered nurse or physician assistant in the state of Ohio.
  • Minimum one year experience as an advanced practice provider preferred.
  • Successful completion of orientation program upon hire.
  • BLS certification.

Responsibilities

  • Collaborate with the Medical Director to support and direct staff within the Internal Medicine Center.

  • Develop and advance the quality of advanced practice through specialized direct patient care.
  • Lead consultation, education, and research efforts.
  • Improve processes of care for patients.
  • Maintain clinical competency through a clinical caseload.
  • Coordinate services with physicians, care managers, social workers, and other community agencies.
