About The Position

The Advanced Practice Provider (APP) will be a key member of the Primary Care Team at UBMD Internal Medicine, responsible for evaluating patients with acute problems, performing minor procedures, and participating in departmental activities as assigned. This full-time position emphasizes health promotion, disease prevention, and quality patient care through effective communication and collaboration with the clinical team.

Requirements

• Current NYS License as Physician Assistant or Nurse Practitioner.
• Minimum two years' experience with healthcare delivery, preferably in primary care and/or family medicine.
• Excellent communication and attention-to-detail skills.
• Experience with electronic health records.

Responsibilities

• Examine patients to obtain information about their physical condition.
• Initiate the evaluation of patients with acute problems, gathering appropriate data through history and ordering necessary tests.
• Instruct and counsel patients about prescribed therapeutic regimens and health maintenance.
• Participate in departmental conferences and quality assurance efforts as determined by the Medical Director or Division Chief.
• Maintain patient flow and maximize the delivery of quality care as a member of the clinical team.
• Emphasize health promotion and disease prevention through patient education and care coordination.
• Review and address all assigned items related to documentation and results within the EHR.
• Complete all documentation and billing related to encounters within 5 business days from the date of service.
• Assist with tracking or documenting quality measures for identified payers or programs.
• Discuss concerns with the Collaborating Physician, Medical Director, and/or Division Chief in a timely manner.
• Follow regulations and guidelines for writing prescriptions.
