About The Position

The Hospitalist APP is an integral member of the inpatient medical team at AdventHealth Porter Hospital, providing high quality, compassionate, and coordinated patient care. This role primarily focuses on the comprehensive management of orthopedic surgical patients in collaboration with the orthopedic surgical team, nursing, care management and other ancillary teams. The APP will primarily assess, diagnose, and manage post-surgical orthopedic patients, facilitate multi-disciplinary rounds, and aid in discharge planning for optimal recovery and healing. The position supports the AdventHealth ministry of Christ through whole person care- addressing the physical, emotional and spiritual needs of every patient. GENERAL SUMMARY: Collectively, the AdventHealth Medical group Hospital Medicine Program provides patient-centric care at our 5 hospitals in the Denver Metro area.   • Graduate of an MSN or DNP Nurse Practitioner Program
  • Graduate of a Physician Assistant, PA-C Program
  • Full Federal and State Prescriptive Authority
  • Eligibility to work in the United States
  • Current ACLS Certification
  • Possess or have the ability to procure and maintain a Colorado Medical License
  • Current DEA Licensure
  • Active member of the hospital medical staff and/or employed by the hospital.
  • Performs duties and interacts with others in a professional manner.
  • Maintains good working relationships with all coworkers.
  • Establishes and maintains a history of regular attendance; makes appropriate use of PDO and observes department call-in procedures for absence; establishes and maintains punctual work habits. Exhibits timely arrival and departure and dependable time habits including meal and other breaks.
  • Attends and participates in mandatory facility-wide and department training/meetings as required (including but not limited to: ALN, safety training, etc.). Is able to demonstrate and apply knowledge of fire, safety, security, and disaster procedure regulations as presented in orientation, outlined in the safety manual, and as pertains to each work area.
  • Required to respond to emergency situations (i.e. disasters, hurricanes, etc.) by reporting to department and staying until the crisis is over or your position is covered by incoming personnel. This is a mandatory requirement. Refusal to respond may result in termination.

Responsibilities

  • Provides daily rounding, assessment and management of post operative orthopedic patients in collaboration with orthopedic surgeons/APPs. May flex to other care areas/units that Hospital Medicine covers if the Orthopedic surgical schedule/volumes decrease to optimize value
  • Manages orthopedic consultations in a timely manner
  • Evaluates and manage medical comorbidities and post-surgical complications to optimize recovery and outcomes
  • Participates in multidisciplinary rounds to ensure effective communication, discharge planning and timely patient throughput
  • Accepts transfers of care from the orthopedic service lines if the patient is requiring increased medical management post operatively
  • Responds and participates in Rapid Responses and Code Blue as needed
  • Participates in staff education through various education modalities
  • Participates in committees and meetings as needed or assigned
  • Responds to clinical, documentation queries in a timely manner
  • Maintains current understanding of regulatory environment
  • Encourages in network utilization of resources for continuity of care unless otherwise specified by patient or physician
  • Participation as needed in quality improvement initiatives and performance metrics
  • Supports effective hospital throughput with pt management
  • Monday through Friday schedule, inclusive of evenings and holidays based on facility and hospital medicine program needs
