The Family Support Case Manager will provide resources for care, collaborate with a multidisciplinary team, and encourage healthy choices as clients transition to outpatient therapy, and treatment. The Family Support Case Manager will serve as an advocate for families of AOT clients and will utilize needs assessment in service planning and crisis support. This is a full-time, salaried positions that works Monday-Friday, 7:30am-4:30pm.
