Aortic Center Registered Nurse Navigator position in the Cardio Thoracic Surgery department in Danbury, CT Full time, Monday- Friday 40 hours per week, 8 hour shifts. Summary: The Aortic Center Nurse Navigator is a specialized Registered Nurse responsible for coordinating the complex care of patients with aortic diseases. The Nurse Navigator coordinates multidisciplinary teams, facilitates timely appointments, educates patients and families, and removes barriers to care, acting as the primary point of patient contact to ensure a seamless continuum of care for the patient. This role reports directly to the Aortic Center Program Director. Requirements

  • Registered Nurse (RN) license
  • Bachelor of Science in Nursing (BSN) required.
  • ACLS and BLS certification
  • At least three years of clinical experience in critical care or cardiovascular procedures/surgery.
  • Strong communication, organizational skills, critical thinking, and the ability to lead in a multidisciplinary environment.

Responsibilities

  • Patient Navigation: Serves as the main contact point, guiding patients through the continuum of care (pre-procedure, hospitalization, discharge, and follow-up). (25%)
  • Clinical Coordination: Coordinates with physicians and specialists for complex aortic aneurysm/dissection cases, including managing surveillance programs. (20%)
  • Education and Advocacy: Educates patients/families on treatment options, disease processes, and self-care. (15%)
  • Operational Support: Tracks patients, reduces hospital readmissions, and ensures adherence to follow-up. (15%)
  • Clinical Triage: Acts as a liaison for clinical questions, triaging patient calls and adjusting medications under physician direction before escalating to advanced practitioners. (15%)
  • Quality Improvement: Collaborates with the multidisciplinary team to maintain patient registries and track clinical outcomes for program growth. (10%)
