About The Position

The Adult Outpatient Case Manager responds to and develops initial crisis management and recovery plans for clients. The ideal candidate will embrace excellent customer service with resource information and connecting clients to immediate access of services reducing the barriers to treatment. In this dynamic role the Adult Outpatient Case Manager will also regularly provide early screening and intervention; case coordination; urgent next day follow-up and triage appointments. Our mission is to empower individuals, families, and communities by promoting hope, healing, and support. NKHS is a Designated Agency contracted with the State of Vermont to provide the highest quality of service in the areas of adults with intellectual/developmental disabilities, children and youth with serious emotional disabilities, and adults with mental health and substance use challenges. We serve the populations of Caledonia, Essex, and Orleans counties through a wide variety of targeted programs for individuals of all ages. NKHS has over 500 employees providing case management, community and home supports, residential care, psychiatry, medication management, therapy, vocational supports, school based counseling, emergency care, and respite services. Requirements

  • Bachelor’s degree in Human Services, Psychology, Addiction Studies or a related field preferred, but life experience and some education will be considered.
  • Must have or be willing to obtain Recovery Coach Training.
  • Experience working in alliance with clinical and mental health services, developmental services and/or substance abuse strongly preferred.

Responsibilities

  • Provides immediate response upon referral receipt to provide: excellent client service; resource information; expeditious access to services; identify and reduce barriers; increase problem solving; develop initial treatment plan goals; early screening and intervention; case management; urgent next day follow-up and triage appointments.
  • Understanding of community and agency resources and the ability to make appropriate referrals.
  • Conduct early screening, intervention, and crisis counseling, including risk assessment and triaging immediate needs.
  • Provide comprehensive case management, coordination, and referral services.
  • Maintain accurate clinical records and documentation.
  • Actively participate in clinical team meetings to inform and coordinate services.
  • Consultation and educational services to the community, area agencies, professionals, and schools as required.
