About The Position

Our Clinical Pharmacy team is looking for a Anticoagulation Clinical Pharmacy Specialist to join our team! As a Clinical Pharmacy Specialist at Memorial Sloan Kettering, you will help advance our mission by providing high-level anticoagulation stewardship, collaborating across disciplines, and improving patient safety, quality, and regulatory compliance. MSK’s Anticoagulation Management Stewardship Program is being developed to address the high-risk use of anticoagulants across inpatient, outpatient, urgent care, and procedural settings. The role will support standardized anticoagulation care, real-time clinical decision-making, quality improvement, education, and data-driven monitoring. In this fully clinical role, you will serve as an integral member of the anticoagulation stewardship effort and partner with the Benign Hematology Service, physicians, nursing, and informatics teams. Core responsibilities include anticoagulation therapy review, prospective audit and feedback, and development of evidence-based protocols and clinical decision support tools.

Requirements

  • Doctorate in Pharmacy is required
  • NYS license (or eligibility)
  • Completion of an ASHP-accredited PGY1 pharmacy practice residency
  • Strong clinical judgment, independent work style, and comfort with interdisciplinary collaboration are important for this role

Nice To Haves

  • Completion of an ASHP-accredited PGY2 specialty residency preferred. Preferably in area of specialty practice they are applying for OR at least 2 years of relevant pharmacy practice experience in specialty area

Responsibilities

  • Provide daily review of anticoagulated inpatients, including patients receiving therapeutic UFH and enoxaparin
  • Lead or contribute to development and approval of anticoagulation guidelines, algorithms, and SOPs.
  • Review anticoagulation-related safety events, medication errors, adverse drug reactions, and HIT-related events to drive continuous improvement
  • Drug therapy design and monitoring and participation in Collaborative Drug Therapy Management (CDTM) initiatives
  • Precepting PGY2 pharmacy residents and pharmacy students
  • Educate providers, nurses, trainees, and patients/families on anticoagulation best practices, medication adherence, and laboratory monitoring
