Summary Conducts subsequent patient visits in accordance with CMS Medicare Annual Wellness Visit guidelines. Works collaboratively with the clinician and care team to provide telephonic and face to face health risk assessment, review the individual's functional ability and level of safety means, provide education, counseling and support to eligible Medicare insured patients.

Requirements

  • Graduate of an accredited school of nursing.
  • 2 years experience as a Registered Nurse.
  • Ability to initiate, accept, and adapt to constant change within a healthcare setting.
  • Demonstrates mature emotional intelligence.
  • Excellent computer skills including electronic medical record (EMR), word processing (Microsoft Word), PowerPoint, and Exel.
  • Knowledge of medical terminology.
  • Ability to multitask and change priorities rapidly.
  • BLS - Basic Life Support - American Heart Association Upon Hire
  • RN - Licensed Registered Nurse_PA - State of Pennsylvania Upon Hire

Nice To Haves

  • Bachelor’s Degree in nursing.
  • 2 years experience in a primary care or ambulatory setting.

Responsibilities

  • Conducts subsequent patient visits in accordance with CMS Medicare Annual Wellness Visit guidelines.
  • Works collaboratively with the clinician and care team to provide telephonic and face to face health risk assessment, review the individual's functional ability and level of safety means, provide education, counseling and support to eligible Medicare insured patients.
  • Integrates clinical expertise with patients’ and families’ values and preferences to provide holistic, coordinated care in primary settings, emphasizing education, prevention, and wellness.
  • Works closely with primary care providers and inter-professional teams to ensure seamless communication, mutual respect, and shared decision-making to optimize patient outcomes.
  • Implements care pathways, follows established protocols, uses data for quality improvement, and leverages technology to enhance safety, reduce risk, and support ethical decision-making.
  • Oversees and documents patient interactions and care activities accurately in the EMR and ensures compliance with regulatory standards.
  • Commits to professional growth, fosters an inclusive environment, actively participates in team discussions, and embraces feedback to enhance practice.
  • Actively supports in-basket work, patient triage, or other practice related needs as business needs determine.
