About The Position

As an Ambulatory RN Care Manager you’ll be responsible for exemplary cross-continuum care coordination, orchestrating care across the health care system and identifying opportunities to improve patient care, services and overall population health. You’ll employ the nursing process in the delivery of patient-centered nursing care and work collaboratively with multiple disciplines and resources facilitating wellness, and transitions of care for patients in the ambulatory environment. Requirements

  • Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreement
  • Current Registered Nurse licenses in Washington State or Multistate license endorsement (MLS)
  • Master’s degree in nursing preferred
  • Certification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire ( E mployees hired prior to 8/1/2023 will have this requirement waived)
  • Three (3) years of recent clinical hospital/ambulatory experience
  • Knowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferred

Responsibilities

  • Ensure the patient and family experiences seamless and safe transitions of care and engage with them as an integral and essential component of the care delivery process
  • Advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefs
  • Provide patient-centered education of disease processes and wellness opportunities
  • Assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as needed
  • Collaborate with leadership and the care team to identify opportunities, and develop a plan of action to promote appropriate utilization of resources across the continuum
  • Serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.
