The Ambulatory Renal Transplant Clinical Dietitian serves as an integral member of the interdisciplinary transplant care team. This role focuses on providing comprehensive nutrition care to renal transplant patients in the outpatient setting through a hybrid work model that includes both in-person clinic visits and remote care. The dietitian conducts individualized nutrition assessments, develops and implements tailored nutrition care plans, and provides ongoing monitoring and follow-up to evaluate patient progress. Nutrition interventions are adjusted as needed to support optimal transplant outcomes and long-term patient health.
