About The Position

In collaboration with the patient and the patients' health care team, the ambulatory pharmacist clinician works to optimize the patient's drug therapy to improve the quality of life and health outcomes of the patient. The standard of care includes ensuring the patient’s medications are appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. The position requires high level of clinical skills, and the ability to build positive relationships with patients and colleagues, including other health care providers. Essentia Clinical Pharmacy is looking for the right candidate to open a new practice in one of our rural communities, either the Baxter Clinic or Virginia Clinic.

Requirements

• Graduation from an ACPE accredited pharmacy program or equivalent.
• Completion of a PGY1 or PGY2 ambulatory care residency or at least 2 years of experience in ambulatory clinical pharmacy.
• Board certification or eligibility to become board certified within one year.
• Active license to practice pharmacy in MN and WI.
• BLS certification within 90 days from hire date.

• Completion of an ACPE accredited Medication Therapy Management training program.
